What is the definition of medical specialty practice?
Medical specialty practice is a compulsory didactic activity included in Medicine Faculty’s Curriculum, starting with the first year of study, until the 5th year of study.
According to education program, the number of hours for summer practice is 90 hours for the first year and 100 hours for the 2nd, the 3rd, the 4th and the 5th year of study.

When is carried out the medical specialty practice?
Medical specialty practice takes place between 15.07-15.08 or between 15.08-15.09 2015.
SUMMER PARCTICE IS NOT ALLOWED DURING UNIVERSITY YEAR.

Where does medical specialty practice take place?
The first year students and the 5th year students have the clinical practice in General Practitioner's consulting room.

The medical specialty practice for the students form 3rd and 4th year of study is carried on in the clinical departments within medical field in Cluj or in home town, provided that an agreement is signed between „Iuliu Hațieganu” Medicine and Pharmacy University and the health unit. The specialities included in this category are : internal medicine, cardiology, gastroenterology , hepatology, neurology , psychiatry, hematology, pneumology , nephrology , diabetes, eating disorders , endocrinology , infectious diseases , dermatology , paediatrics, medical oncology , rheumatology , medical rehabilitation , geriatrics, ICU, emergency medicine, allergy - immunology, clinical pharmacology.

The medical specialty practice for the students form 3rd and 4th year of study is carried on in the clinical departments within surgical field in Cluj or in home town, provided that an agreement is signed between „Iuliu Hațieganu” Medicine and Pharmacy University and the health unit. The specialities included in this category are : general surgery , surgical oncology, orthopaedics, traumatology, neurosurgery, ENT, paediatric surgery , ophthalmology, cardiovascular surgery , obstetrics and gynaecology , thoracic surgery , urology , plastic surgery .

Which are the educational goals for medical specialty practice?
Educational goals for medical specialty practice are the following:

First year
At the end of summer practice, the students will be able to:
 observe and analyze morbidity issues identified in the consulting room’s appropriate area
 know the main preventive measures
 identify environmental risk factors
 prepare materials for medical education
 organize activities with an educational purpose
 analyze the health condition of the population from the area
 summarize a patient 's health problems
 observe and become familiar with patient interviewing, conducting medical consultation, diagnosis, establishing a treatment plan
 effectively communicate with the doctor, with the nurse or the patient


Second year
At the end of the summer practice, the students will be able to:
 know hygiene rules from a hospital,
 know the medical procedures’ steps from a hospital ward within medical field
 become familiar with the procedures : patient questioning , information analyzing, clinical examination , diagnosis, treatment purpose and periodic auscultation
 prepare the patient for current laboratory explorations
 harvesting biological materials (urine, feces, throat swab)
 observe and participate in the administration of oral and inhaled treatment
 communicate effectively with the nurse, develop teamwork skills
 know the general principles of patients’ nutrition
 familiarize themselves with prevention procedures of public health risks

3rd year
At the end of the summer practice, the students will be able to:
 perform medical history and physical examination in hospitalized patients
 establish electronic clinical record for the patients
 formulate clinical diagnosis of the syndrome or disease and to conduct a plan of medical investigations
 perform manoeuvres in order to establish a diagnosis
 interpret the results of current investigations
 inform patients and prepare them for clinical and laboratory explorations
 analyze and correctly describe the therapeutic management of patients with medical diseases and conditions
 know the medical procedures’ steps from a hospital ward within medical field
 know emergency care manoeuvres carried out at the hospital ward where the internship is taking place
 observe the care provided in the moment of receiving medical emergencies
 Know the written medical documents and computerized medical records required in patient’s management

4th year
Specific goals
At the end of the summer practice, the students will be able to:
 conduct patient history and clinical examination at surgical patient
 establish electronic clinical record for the patient
 formulate clinical diagnosis of the syndrome or disease and to conduct a plan of medical investigations
 interpret the results of current investigations
 make dressings and bandages, administer a vaccine used to prevent tetanus (DTaP, Tdap, DT, Td)
 prepare the patient for current laboratory explorations
 analyze and correctly describe the therapeutic management of patients with surgical diseases
 establish a treatment plan
 know the medical procedures’ steps from a hospital ward within surgical field
 know the rules for asepsis and antisepsis
 know emergency care manoeuvres carried out at the hospital ward where the internship is taking place
 Know the written medical documents and computerized medical records required in patient’s management

5th year
At the end of the summer practice, the students will be able to :
 carry out preventive consultations
 make patients’ auscultation, together with the doctor, both in consultation room and at patient’s home
 establish laboratory diagnostics using all available methods
 recognize common diseases and to establish appropriate therapeutic protocols
 establish comprehensive treatment plan for patients
 observe the approaching of a patient with chronic pain
 advise the patient to go to other medical units for specialist physicians’ consultation
 recognize common paediatric diseases and identify a sick child
 recognize mental health problems in patients presenting to family medicine cabinet
 identify environmental risk factors
 analyze and report the health condition of the population from the territory
 assess healthcare costs


Which are the manoeuvres the student must learn during medical specialty practice?
Medical manoeuvres which each student must learn during clinical practice are the following :
First year
 patient identification according to his health care card
 Observing and performing anamnesis
 Observing and performing physical examination
 Determination of weight and waist
 Measuring vital signs (pulse, blood pressure, temperature)
- Monitoring chronic patients
- Patient communication on health education measures

Second year
- Hospitalized patient’s procedures
- Harvesting urinalysis, sputum, faeces, pharyngeal exudates
- Interpretation and explanation of a urinalysis, complete blood count (CDC)
- Determination of capillary glycaemia
- Administration of the oral and inhaled medications
- Performing EKG
- The administration of oxygen therapy (method of administration)

Third year
- Venous blood collection
- Performing EKG
- Informing patients to carry out explorations (abdominal ultrasound, endoscopy, bronchoscopy, radiography with contrast, stress test)
- Patient preparation for invasive explorations (endoscopy, radiography with contrast, biopsy)
- Recording patient’s evolution in the clinical record
- Discussion of a treatment plan
- Writing a medical prescription
- Explaining the therapeutic recommendations to the discharged patient

Fourth year
- Surgical hand washing with water and disinfectants
- Cleaning surgical wounds
- Making dressings for wounds or surgical incisions
- Communication with the patient regarding medication side effects
- Rules and methods of disinfection, asepsis and antisepsis
- Venous blood collection
- Intramuscular injection
- Subcutaneous injection
- Supervision of pre- and postoperative surgical patients (blood pressure, temperature, pulse, respiration, diuresis, bowel transit, vomiting, surgical drainage)
- Specific manoeuvres for clinical departments in which the activity is carrying on

Fifth year
- Peculiarities of drug administration in children
- Measuring children’s blood pressure
- Filling patient’s electronic record
- Knowing child’s history of vaccinations / Knowing adolescent’s history of vaccinations
- The explanation of the treatments to the patients / to the parents (effects, side effects)
- Filling in the specific forms and documents, under guidance (medical referral letter for patient.)
- Following the pregnancy growth (the frequency and the content of prenatal consultations)
-  Sending the patient to specialist physicians for consultation

How many credits are required to pass the medical specialty practice?
For this activity the students will receive 2 credits per year.

How is assessed the medical specialty practice?
The grading system is based on pass / fails scale. The pass/ fail grading is established by a committee approved by Medicine Faculty’s Board. The pass/ fail grading is given from the 25th of September until the 10th of October.

What documents must be submitted for the evaluation of medical specialty practice?
Each student will submit to the year secretary until the 25th of September, the following documents:
1. Manoeuvres Checklist - 1st year2nd year, 3rd year, 4th year, 5th  year
2. Activity portfolio - will include the description of 10 clinical cases pursued by the student during medical specialty practice (history, anamnesis, physical examination, laboratory tests, diagnosis, treatment, evolution history). More about portfolio you can find here
3. The evaluation sheet must be filled in and signed by the head of department for outside Cluj hospitals and for Cluj clinics by the tutor appointed by Medicine Faculty’s Board. The list of appointed tutors can be found here.
